![]() The written request to change health plans must include the following information: Requests should be directed to the health plan or a member can provide written request can be submitted and it will be forwarded to the member’s current health plan. If the member’s concern can be handled by another health plan, the member will receive a response informing them of their ap aproval to change health plans. If the member’s issue cannot be resolved by their health plan, the member will receive a response informing them their issue cannot be resolved. Members should contact their health plan for resolution to any of the concerns listed below, but is not all-inclusive.
